Four Levels of Training in a Day!

Putting on a "blitz" is only one of the things that the Southeast AETC (SEATEC) does well. Dianne Weyer, clinical instructor at SEATEC, offers Intensive On Site Training, a multifaceted training that utilizes formats including a Level I, Lunch and Learn for all staff; Level II case studies for clinical staff; and Level III and IV one-on-one modeling of behavior for healthcare professionals. Technical support for nurses and administrators includes reviewing and establishing flow sheets, assuring current PHS guidelines and organizing patient scheduling. Focusing on rural communities, SEATEC is able to customize programs based on the site assessment of the clinics' resources.

SEATEC staff have broad knowledge of the varied structures of rural clinic settings and how different Title III programs are organized. In addition to geographic barriers, a rural community providing care for patients in a Ryan White setting faces limited resources. To implement training, SEATEC begins with careful planning, contacting administrators and clinicians by telephone or by e-mail. The clinic staff identify their goals for the SEATEC visit and prepare by pulling charts and scheduling patients that they want to see with the SEATEC clinical instructor. As the clinical instructor, Dianne recognizes that "it's hard to air your dirty laundry in front of a stranger" and acknowledges that most clinics are doing the best they can with what they've got. She points out their strengths and makes suggestions for areas that need improvement. To facilitate this evaluation, both clinical and non-clinical staff complete a brief clinical test with a couple of case scenarios. Staff members are often surprised at how much they know, and open to learning about the areas where they lack information.

The main obstacle has been that clinic staff, eager for information, are unrealistic about how much instruction can be provided to them in one visit. SEATEC routinely provides educational resources and guidelines, information on the local performance site of the AETC, warmlines, and the National Resource Center. Many times a follow-up visit is arranged to allow for additional information sharing. A major strength of the SEATEC on-site training is that it is "grounded in the real world."
