Chart Review for Targeted Training

One of the innovative education strategies of the Florida/Caribbean AETC for Title II programs is utilizing chart review as a needs assessment for targeted training activities. The chart review focuses on how successful the Title II program is in meeting the medical standards of care. Led by Dr. Jeffrey Beal, the chart review is a team effort consisting of both physicians and nurses. Ideally, individuals from other Title II programs are included in this educational opportunity.

A Noon Case Conference follows the morning chart review. The Title II faculty and staff are encouraged to present one or two challenging teaching cases or cases representing a common theme to the team.

After the Case Conference, the team meets with the clinic manager, physician, nursing director, office manager, and other clinic staff. The strengths and weaknesses of the program are discussed. Emphasis is placed on the role of the AETC as a resource in reaching these goals either by providing training, tools, or offering skills-building exercises. For example, lack of physician services is one of the common obstacles for Title II programs. It is not unusual for one physician to see 60-80 patients during his/ her monthly visit to the clinic. To bridge the continuum of care, nurses play a critical role in ensuring patients' quality of care in the interim. Since this is a new role for some nurses, the chart review team helps the nurses develop strategies for monitoring care between visits.

The chart review team documented positive changes in the healthcare services during their annual visit to the Title II programs. The Florida/Caribbean AETC focuses on outcome measures to document these changes and has developed a checklist to collect outcome data (available at the Florida/Caribbean website www.faetc.org). The Florida AETC welcomes comments and input from others using this resource.
