The Longitudinal Training (LT) Model was developed by the New Jersey Local Performance Site (LPS) of the New York/New Jersey AETC. The New Jersey LPS, located at the Division of AIDS Education of the University of Medicine and Dentistry of New Jersey (UMDNJ), combined AETC and Minority AIDS Initiative (MAI) resources to enhance the HIV care capacity of community-based healthcare centers that serve predominantly minority populations. The LT Model is a flexible, multidisciplinary, highly adaptable training framework that targets clinical and non-clinical healthcare providers through comprehensive needs-based programs. Site-specific needs assessments help determine gaps in HIV clinical knowledge, cultural competency, case management approaches, and continuous quality assurance applications. The resulting data are then used to develop individualized LT workplans. The New Jersey LPS LT Model focuses on targeted on-site training, off-site preceptorships, didactic conferences, and group case discussion. These varied opportunities facilitate clinical training of healthcare professionals who provide HIV care for LT Site patients, but are not necessarily integrated within healthcare clinics-eg, oral health professionals and community pharmacists. Through LT clinical preceptorships, pharmacists and oral health professionals participate in a multidisciplinary team approach to HIV care. Outcome evaluations demonstrate how the LT Model enabled the NJ LPS to significantly increase the number of clinicians trained through Level III (Clinical Preceptorship) and Level IV (Individual Clinical Consultations) modes of learning. The LT Model is easily adaptable to fit differing training needs and healthcare infrastructures within other AETC regions.
