Description:

The Patient Satisfaction Survey consists of a core survey, which is applicable to all HIV-positive patients with HIV primary care visits, and five modules (case management, outpatient substance use services, mental health services, women's health services, and Medicaid managed care). Both, the core survey and all five modules are available in English and Spanish. Additionally, the document include a provider guide that will assist HIV programs to administer the survey by providing some guidance on sampling, administering the survey, and analyzing the results.
