To Submit Patient Education Materials

Patient education material may be submitted for review to the AETC National Resource Center at the New York State Department of Health AIDS Institute (NYSDOH/AI).

Printed materials may be mailed to:
NYSDOH AIDS Institute, ESP Corning Tower, Room 259, Albany, NY 12237-0658 Attention: Ms. Jennifer Parr, M.P.H., NRC Coordinator. Please download the submission form and send the completed form along with your submission to the address above.

Electronic submissions:
Materials may also be submitted electronically to Ms. Jennifer Parr at: aetcnrc@health.state.ny.us Materials may be submitted in either Word, WordPerfect, PowerPoint, HTML, or Adobe Portable Document File (.PDF) formats. Please download the submission form and send the completed form along with your submission to the e-mail address above.
