Vertical Transmission of HIV-1 Variants Resistant to Reverse Transcriptase and Protease Inhibitors [De Jose, MI et al. Arch Intern Med 2001;161:2738] The authors from Spain describe the mutation patterns of two patients with maternal-fetal transmission of HIV. In the first patient, who did not know about HIV infection and was untreated, the maternal and infant strains of HIV showed identical resistance mutations, including RT mutations at codon 184 and 215, and protease mutation at codon 63. The interpretation was resistance in the transmitted strain to the mother with subsequent transmission to the infant. The second patient had advanced AIDS with noncompliance and multiple drug exposure. The maternal strain showed RT mutations at codon 215 and 219, but the infant showed only the 215 mutation. The authors conclude that this type of study of perinatal transmission can provide information about natural selection of subpopulations and that this might be useful in guiding therapy in the potentially infected offspring.
