Generalized Tuberculosis in the Acquired Immune Deficiency Syndrome [Smith MB, et al. Arch Path Lab Med 2000;124:1267]: The authors report autopsy findings of 20 fatal cases of disseminated TB. The diagnosis was made at autopsy in 50%, and only 55% had fever >38°C. Major autopsy findings were thoracic and abdominal lymphadenopathy in 95%, necrotizing encephalitis in 100%, brain abscess in 50%, and peripheral adenopathy in 45%. Lung involvement was noted in 90%, but 33% were reported to have normal chest x-rays. The histologic findings in most organs showed acute inflammatory foci with extensive necrosis with many polys and no giant cells.
Comment: This report emphasizes that patients with AIDS may develop disseminated tuberculosis that has unusual clinical features. We know this, but the present report emphasizes the degree of atypical features by clinical parameters as well as pathologic criteria.
