Despite High Concordance, Distinct Mutational and Phenotypic Drug Resistance Profiles in Human Immunodeficiency Virus Type 1 RNA are Observed in Gastrointestinal Mucosal Biopsy Specimens and Peripheral Blood Mononuclear Cells Compared with Plasma [Poles MA, et al. JID 2001;183:143]: The authors examined genotype and phenotype resistance patterns of HIV RNA isolated from the colonic mucosa in seven patients who underwent sigmoidoscopy. Results were compared with resistance tests in HIV isolates from plasma and from PBMC. There was a high degree of concordance by both genotypic and phenotypic analysis, although three patients showed mutations from endoscopic biopsy specimens that were not seen in plasma isolates.
Comment: The relevance of this study concerns the concept of independently evolving quasi species in compartmentalized HIV. This appears to be the first study involving the GI compartment, which accounts for about 25% of the lymphocyte pool. Despite the high level of concordance, there were important exceptions since three of the seven patients showed single mutations in GI mucosa-derived HIV RNA or PBMC not seen in plasma strains.
