Pulmonary
Pathology in Patients with AIDS: An Autopsy Study from Mumbai [Lanjewar
DN and Duggal R. HIV Medicine 2001;2:266] This
is an autopsy study of 143 patients with HIV infection with a focus
on the spectrum of pulmonary lesions. The results showed pulmonary
pathology in 126 (88%) including TB in 85 (59%), bacterial pneumonia
in 26 (18%), CMV in 10 cases (7%), cryptococcosis in 8 (6%), and
P. carinii in 7 (5%).
Comment: The unusual finding in this case series compared
with other reports is the paucity of PCP. The potential explanations
for the disparate results compared to findings in industrialized
countries include: 1) predominance of other pathogens such as tuberculosis,
2) different epidemiology with sparse rates of P. carinii,
3) lack of diagnostic tests to detect PCP, and 4) earlier death
at a stage of disease that does not yet predispose to PCP. This
study certainly supports the concern for high rates for TB.
