Prophylaxis
with a nevirapine-containing triple regimen after exposure to HIV-1
[Benn PD, et al. Lancet 2001;357:687]: The
investigators from London report their experience with nevirapine-containing
regimens for prophylaxis after occupational or sexual exposure to
HIV by 57 patients. The follow-up showed that five developed grade
3-4 drug-induced hepatitis including two with a rash. The authors
conclude that, despite the biological rationale for this regimen,
safety issues make it unacceptable where alternative regimens are
available.
Comment: There have been substantial reports recently about
nevirapine-associated hepatic injury. Thus, this report does not
come as a surprise, but simply adds to the weight of prior concerns.
