Symptomatic Hyperlactataemia: An Emerging Complication of Antiretroviral Therapy [Gerard Y, et al. AIDS 2000 14:2723]: The investigators from France report observations in 14 patients with systematic hyperlactataemia. All had arterial lactate levels above 1.5 mmol/l; the median was 2.4 mmol/l. There were elevated transaminase levels in seven, and liver biopsies showed steatosis in three of four. Symptoms were variable, but the most common were fatigue, weight loss, and exercise-induced dyspnea. These 14 patients were from a population of 964 for an incidence of 0.8%/year; all 14 patients had received d4T, and the incidence in this group was 1.2%/year. These patients improved when NRTI therapy was discontinued.
Comment: : This is another of several recent reports dealing with lactic acidosis as a complication of NRTI therapy. The favorable outcome in this series was attributed by the authors to early detection.
