The Effect of Treatment of Vaginal Infections on Shedding of Human Immunodeficiency Virus Type 1 [Wang CC, et al. JID 2001;183:1017]: This is a report from Kenya on the effect of treating vaginal infections on HIV shedding. The assay method was vaginal swabs using Gen-Probe, a method that has shown good correlation with spiked samples and with measurements of plasma HIV viral load levels. For women with Candida vaginitis, treatment was associated with a reduction in viral load from 3.36 to 2.86 log10 c/swab. For treatment of Trichomonas, the reduction was 3.67 to 3.05 log10 c/swab. For bacterial vaginosis, there was no change.
Comment: These studies show a three- to four-fold reduction in viral load with treatment of Candida or Trichomonas vaginitis. The assumption is that this will reduce the probability of HIV transmission with sexual intercourse or with perinatal transmission.
