Lipodystrophy: A Review
[Kravcik S. HIV Clin Trials 2000;1:37]: The author reviews
lipodystrophy and shows a proposed definition based on a prior report
by Carr A, et al. [Lancet 1999;353:2093]:
- One or more of the following based on patient report or physical
exam:
- Fat wasting of arms, legs, face, and/or buttock.
- Central obesity involving abdomen, dorsal spine (buffalo hump),
and breasts.
- One or more of the following metabolic changes:
- Fasting triglycerides >174 mg/dL.
- Fasting total cholesterol >214 mg/dL.
- Fasting C-peptide >7.5 mg/mL.
- Glucose intolerance with a fasting glucose of 110 - 126 mg/dL,
impaired glucose tolerance at 140 - 240 mg/dL, or diabetes
with fasting glucose >126 mg/dL.
- No active AIDS-related complication.
- No current steroid or immune modulator therapy.
- Diagnosis: Standard anthropometric measurements (waste:hip ratio,
skinfold thickness, weight and height) or imaging [ultrasound, CT of
the abdomen, and VAT:TAT ratio (visceral adipose tissue: total adipose
tissue)].
