HIV-Associated Hematologic Disorders are Correlated With Plasma Viral Load and Improve Under Highly Active Antiretroviral Therapy [Servais J et al. JAIDS 2001;28:221] This a two-phase study from Luxembourg to determine the relationship between viral load and marrow suppression, and response of suppression to HAART. The initial phase of this study showed a significant inverse correlation between the HIV plasma viral load and all three hematologic parameters in a sample size of 207. These reduced levels correlated with CD4 cell count. The second phase of this study concerned the response of patients with hematologic alterations before and after HAART. The results showed hematologic reconstitution for all three cell lines, which became statistically significant after 6 months. The results are shown in the following table:

Comment: Many studies have shown that HIV infection is often complicated by marrow suppression which usually occurs in advanced disease, although thrombocytopenia alone is often seen relatively early in the course. The study here shows a good correlation between viral load and marrow suggesting a direct effect of HIV. This study also shows that HAART is associated with significant increases in all three cell lines.
